How to book an appointment
For patients
You will need to ask your GP for a referral letter to access our services. The process for referring is quick and simple and an appointment can normally be arranged to suit you.
Once we have received the referral form for a diagnostic test we will write to you offering you an appointment. If the appointment offered is not convenient, please contact us by email or phone on 01604 885002 to find a more suitable time.
To arrange a consultation appointment, the consultants’ secretary will contact you to make an appointment directly. If you have any queries please do not hesitate to contact The Pavilion Clinic main reception 01604 885002.
For medical professionals
Requesting a test is easy. All you need to do is complete a referral form and fax it to 01604 885004, (we can arrange for a supply of referral forms to be sent to you, please call 01604 885002).
